Setting up an online account/Using Patient Portal

Patient Portal to make an account in the Patient Portal.
If you look to the right, you will start your online scheduling by going to my patient portal appointments tab, however first you have to make an account. This is a separate and more secure site, than my website, for you to put in your information and to set an appointment.
If you do not have an email address, or are filling it out for an individual who does not have an email address, click the box which says “Register a Child or Username”. This will allow you to create an account which does not have an email associated with it.
Once you have typed in all the information click “submit” and it will take you to the login page. (Make sure to note your user name/email and password for future reference.)
After setting up your account there will be an email verification sent to you. Once you click on the email verification link in your email, your account will be officially set up.
Each time you go to the portal, enter your email address, or username and the password you chose, then click “submit”. This will redirect you into your personal patient account.
You will now have access to clinics forms and will be able to electronically sign them. Please fill out the Patient Info, Consent for Treatment, Description of Services, and Arbitration Agreement Forms so I can legally treat you.
“Appointments” tab, lets you see what appointments you have scheduled and you can even cancel them if you need to.
“Messages” tab, allows you to send me a secure message 24/7 (please allow for 2 business days for reply) with private information. Please do not send health related information to my regular email. I do receive notification when you fill out a message from the portal and check them regularly. I can even send a message back to you.
“Settings” tab, allows you to change your password and set up your notifications.

**Before your first private, or group, appointment please login to the patient portal and sign your paperwork. This helps provide the maximum time to your treatment. If you are unable to fill in this information, on the portal, prior to your first appointment please arrive 20 min. early so we may complete these prior to your scheduled time. Otherwise, time will be deducted from overall treatment time.
